Provided your permanent address falls within our practice boundary, you have the right to apply to join our practice. We are unable to keep patients on our list who move out of this practice area.
We work as a partnership and as such, GP’s do not have individual lists of patients that they see alone, although every patient has a “Named Accountable GP”. You can ask for a particular GP to be listed as your doctor and wherever possible we will try to accommodate this. Furthermore, we will try to ensure you can have continuity of care by providing appointments wherever possible with the GP of your choice. However, there may be circumstances where this is not possible.

2. First, download the GMS1 Registration Form, complete and either scan in or take a clear photo of the form both sides, and e-mail to us at email@example.com
Please make sure you have completed all the relevant details on the form. Complete a separate GMS1 form for each person.

When we receive both the GMS1 Registration Form and a New Patient Questionnaire Form for each patient, and have accepted you onto our list of patients, your details will be sent to the Local Health Authority and your medical records will be forwarded to us from your previous GP.
