Provided your permanent address falls within our practice boundary, you have the right to apply to join our practice. We are unable to keep patients on our list who move out of this practice area.
We work as a partnership and as such, GP’s do not have individual lists of patients that they see alone, although every patient has a “Named Accountable GP”. You can ask for a particular GP to be listed as your doctor and wherever possible we will try to accommodate this. Furthermore, we will try to ensure you can have continuity of care by providing appointments wherever possible with the GP of your choice. However, there may be circumstances where this is not possible.
To register with us:
Complete Registration Form for children under 16 years old only
Complete Registration Form for Men over 16 years old only
Complete Registration Form for Women over 16 years old only
Please allow 3-5 days for your registration to be processed by us. When we have accepted you onto our list of patients, your details will be sent to the Local Health Authority and your medical records will be forwarded to us from your previous GP. Therefore without your previous GP details or date of entry to the UK, we cannot process your registration.
