New Patient Registration

Provided your permanent address falls within our practice boundary, you have the right to apply to join our practice. We are unable to keep patients on our list who move out of this practice area.

We work as a partnership and as such, GP’s do not have individual lists of patients that they see alone, although every patient has a “Named Accountable GP”. You can ask for a particular GP to be listed as your doctor and wherever possible we will try to accommodate this. Furthermore, we will try to ensure you can have continuity of care by providing appointments wherever possible with the GP of your choice. However, there may be circumstances where this is not possible.

To register with us:  

1. Please click here to read the registration instructions before you complete our forms.

2. Please complete the relevant registration form, for each person wishing to be registered.

3. Download the relevant Registration Form from below, complete and send to us either as a document attachment to an e-mail or, take a clear portrait size photo of each page of the form, attach the photos to an e-mail and e-mail to us at:

Please try to save and e-mail your document as a Word or PDF document, or .jpeg.  We cannot open other formats such as ‘.pages’ or ‘.odt’ files.

  • Please make sure you have completed all the relevant details on the form. If you are from abroad you MUST complete that section on the form.
  • Complete a separate form for each person.

Due to the volume of  registrations please allow up to 7 working days for your registration to be processed by us.  When we have accepted you onto our list of patients, your details will be sent to the Local Health Authority and your medical records will be forwarded to us from your previous GP.  Therefore without your previous GP details or date of entry to the UK, we cannot process your registration.
