Millcroft Medical Centre is a part of Eaglebridge Primary Care Network (PCN), which along with Earnswood Medical Centre provides care to over 46,000 patients across Crewe.
Primary Care Networks are the newest solution to collaborative working within the NHS with the aim of sharing resources to tackle health demands of the area.
Together both practices work locally with the community mental health teams, social care, pharmacy, hospital and voluntary services to care for local patients.
We have created a new website for the Primary Care Network to keep you, as patients, up to date on practice news, upcoming events and projects and find support for a range of issues.
